ConfirmPath is a Texas Professional Limited Liability Company established in 2006 to offer a fee-based second opinion review of the pathology slides to confirm all types of pathology diagnosis (including malignant diagnosis) on all forms of tissues such as breast, colon, prostate or lung cancers. Types of samples we examine include biopsies, excisions, resections, FNAs (fine needle aspirations), cytopathology preparations, and bone marrow samplings. We also offer second opinion consultation for all Clinical Pathology reports by reviewing chemistry reports, hematology reports, lipid profiles, protein analysis, blood bank reports and others.

Terms & Service Agreement
Terms. In this document the terms ‘you’, ‘your’ and ‘his/her’ refer to you the patient who is requesting our service, the terms ‘we’, ‘us’ and ’our’ refer to the company ConfirmPath PLLC. The term ‘Service’ refers to the service provided by ConfirmPath PLLC by giving you ‘the patient’ a second opinion on your pathology case slides or pathology test reports.
Description of Service.ConfirmPath PLLC Service is designed to provide patients like you with the opportunity to have a second opinion review (consultation) of their diagnostic materials, including glass slides and any other pathology laboratory documents and reports. We would also review any other types of medical reports including but not limited to other special studies, radiographic and surgical report if correlation with these reports is needed. ConfirmPath PLLC will formally review these materials and prepare a written report only on the pathology part/parts. Results will be provided in a formal written pathology report which includes two copies, one mailed to the patient in a confidential U.S. mail envelope and a copy mailed to the original pathology lab (in the US only.) We may also send another copy to your treating physician, if this act is viewed necessary based upon our clinical judgment. ConfirmPath PLLC WILL NOT perform special studies including, but limited to Immunostains, Chemical stains, or other markers or diagnostic tools. Our service includes microscopic and paper report review only. Our goal is to provide you with information so that you and your treating physician can confidently make the best decision in regards of your treatment plan. ConfirmPath PLLC at its own discretion may elect to have your slides reviewed by a third-party specialized pathologist expert in the field of your case; If ConfirmPath PLLC and its pathologists realizes that the complexity of your case requires an ‘expert in the field’ pathologist’s opinion, in such cases you will receive the expert pathologist’s report only, ConfirmPath LLC may also issue its own report. Also, in such cases ConfirmPath PLLC will be responsible for the expert pathologist’s fees. ConfirmPath PLLC will not offer treatment plans. ConfirmPath PLLC recommends that patients obtain such information from their treating physicians. We only confirm Pathology slides and diagnostic test reports. Our service is not intended or established to generate opposite or unreasonable diagnoses for any financial or legal benefits. ConfirmPath does not offer treatment. This service does not guarantee nor does it promise a change in the original pathology diagnosis. This service is established for a second opinion only. ConfirmPath is not responsible for lost mailed material; also we are not responsible for any damages of diagnostic material due to improper packaging or shipping. Patients have to make sure that their pathology slides are properly packaged with plastic/metallic containers with adequate bubble-wrapping. All cases submitted without payments maybe returned to the sender without examination.
ConfirmPath PLLC’s Fees. Payment may be made by the PAYPAL payment system. Patients may also submit a personal check or a money order directly to our address. ConfirmPath PLLC will not contact your insurance company and will not try to obtain reimbursement from your insurance company. The services we provide may or may not be covered by your insurance company. The payment needs to be received before any examination of any case or any reports are issued. The fee for our service is not subject to reimbursement once your case is being studied or examined by ConfirmPath PLLC including paperwork/history review. ConfirmPath PLLC, on patient’s request, may fax results to his/her treating physician if faster service is needed. Patient may also request more than one physician to receive a copy of the report, a separate fee may apply.
ConfirmPath PLLC’ Advertising Policy. Advertisement is not a financial source for ConfirmPath PLLC or its website. ConfirmPath PLLC does not host or receive funding from advertisement.
